Below are various housing forms not included in the myHousing portal. myHousing is your central spot for housing services at MSU Moorhead: apply for housing, dining and learning communities; apply for commuter/staff meal plans; view your booking and roommate assignment; submit a maintenance request for your room; and view parcels awaiting for your pickup.

Moving In & Out Forms

Form	Description
Form Early Arrival Request	Description This form is to request permission to arrive early to the residence halls and apartments.
Form Early Arrival Requests for Teams and/or Departments	Description This form is for coaches and/or supervisors to setup early arrivals for their students.
Form Late Check Out	Description This form is to request a late check-out date and time.
Form Microfridge Rental Application - FULL FOR FALL 2023	Description This form is used to request to rent a microfridge for the academic year.

Roommates & Room Change Forms

Form	Description
Form Double as a Single Room Conversion	Description This form is a request to convert my double room to a single room.
Form Room Change Form	Description This form is to request a room change. Room change requests are reviewed on Mondays and confirmations sent out on Tuesday. Residents have Thursday-Sunday to complete their move.</td style="vertical-align: top;">

School Break Forms

Form	Description
Form Winter Break Occupancy	Description This form is located in the myHousing portal; students need to request to stay on campus during winter break. This form is available November 1; Students must sign up by December 14th to ensure card access use over break.
Form Spring Break Occupancy	Description This form is to request to stay on campus during spring break. This form is available February 1.
Form Summer Storage	Description This form is to request to keep items in your room if you are returning to your exact same room next fall.

Meal Plan Forms

Form	Description
Form Commuter, Faculty & Staff Meal Plan Sign Up	Description This form is to sign up for a meal plan if you are NOT a housing resident or IN ADDITION to your residential meal plan.
Form Pre-Term Meal Plan Change Request Use myHousing portal if prior to term start.	Description This form is to CHANGE your residential meal plan.

Contract Related Forms

Form Approved Release of Contract	Description This form is used for approved release of housing contract: Graduation EuroSpring International or National Student Exchange Study Abroad Military Commitment Practicum/Internship/Student Teaching Transfer/Withdrawal from the University Academic Suspension Not Enrolling for a Term Session
Form Contract Cancellation Request Pre-Move In	Description This form is to request contract cancellation PRIOR to move in.
Form First Year Live-On Campus Policy Housing Exemption	Description This form is to request to be exempt from the first year live-on campus policy.
Form 2022-2023 Returner Special	Description This form is to waive $50 prepayment and lock into the 2022-2023 room rate.
Form How can I get out of my housing contract?	Description For most students, it's not likely you'll be able to break the contract that you signed and will be held to the terms of the Housing & Dining Contract. Please, please, please DON'T sign any off-campus lease unless you receive official written notice of a contract release from Housing. It could mean that you pay two rents! If you do not meet the criteria listed in the Approved Release of Contract (above), you may request a Contract Release Request. To do so, stop in to our Housing Office (Ballard 120) to pick up the form and learn about the process. Please note, if you are currently in the housing contract, it is unlikely that you will be approved to break your housing and dining contract unless you have documentation establishing that something considerable has changed since you entered your contract (i.e. moved on campus).
