Minnesota State University Moorhead (MSUM) is firmly committed to achieving a campus climate free of discrimination, harassment, and violence, where mutual respect and dignity are expected by all community members.

MSUM depends on community members to identify and report behaviors of concern so that the University can provide distressed students and employees with appropriate services and resources. We are all responsible for campus safety. Please report any concerning behavior or suspicious activity using the forms below.

Report Suspicious Activity

If you see something suspicious on campus, please fill out the online form above. You can complete the form anonymously, or if you indicate you would like to be contacted, a Public Safety employee will review your message and contact you if necessary.

Report Harassment, Discrimination or Retaliation

Any individual who believes they have been or are subject to conduct prohibited by the Equal Opportunity and Nondiscrimination in Employment and Education Policy is encouraged to report the behavior to the designated officer using this form.

Report Sexual Misconduct

This form is available for students, employees, or any concerned person for reporting information involving sexual misconduct matters. This includes instances of sexual harassment, sexual assault, dating violence, relationship violence, and stalking. This information is routed to Minnesota State University Moorhead Title IX Coordinator. This information may be used in addressing and disciplining the accused individual.

Report an Incident of Bias

This form is for reporting incidents of bias, intolerance, or intimidation. A bias incident is any discriminatory or hurtful act that appears to be or is perceived by the impacted person to be motivated by race, color, creed, religion, national origin, sex, gender, age, marital status, disability, public assistance status, veteran status, sexual orientation, or familial status. To be considered an incident, the act is not required to be a crime under any federal, state, or local statutes, nor does it have to violate university policy.

Report an ADA Grievance

If you encounter a barrier to access on the Minnesota State University Moorhead (MSUM) campus, you may use this form to report it to our ADA coordinator.

Report Conduct Violations (Non-Academic)

To report alleged violations of the Student Conduct Code, please use the online reporting form available here. Reports are submitted to the Dean of Students Office for review. If you have questions about whether a behavior is considered a violation, please contact the Dean of Students Office.

Tort Claims & Incident Reports

Tort Claims are filed by non-employees (students, guests, vendors, etc.) who are injured or have their property damaged while on the University's premises.
