Financial Aid Forms
At Minnesota State University Moorhead, we understand applying for financial aid can be an overwhelming process. To make things easier, you'll find a list of commonly used financial aid forms and a description of their purpose below. In some cases, you may need to submit additional forms to the Office of Scholarship and Financial Aid along with your FAFSA in order to receive your award package.
Used to authorize MSUM to credit any remaining funds to miscellaneous charges billed to my account, including MSUM Bookstore charges, library fines, parking tickets, late fees, etc.
Used to identify courses and degree program if seeking a second bachelor's degree, teacher certification, or undergoing preparatory work for a graduate program at MSUM
Used for verification of students who are supporting a child during schooling
Application for the Parent Loans for Undergraduate Students (PLUS) and Grad PLUS
Used to appeal your financial aid suspension
Used to certify you will use financial aid only for educational purposes and to pay the cost of attending MSUM
Used to request consideration of changing your dependency status to independent
Used to request consideration of changing your dependency status to independent due to a marriage date after submitting your FAFSA
Used to verify MSUM can release your information to specified individuals
Used to order a transcript or other tax return information free of charge
Used to order a tax return transcript free of charge
Used to satisfy the Student Educational Loan Fund (SELF) program requirement of the Maximum Effort Test when a FAFSA will not be filed
Used when not filing a FAFSA and requesting a private loan (not MN SELF or Bank of ND DEAL)
Used to appeal the consideration for being awarded an academic scholarship or upgrading your scholarship
Used to request an update to financial conditions or income reported on your FAFSA
Used to appeal the standard budget due to extraordinary expenses incurred during the academic year
Application for the Teacher Education Assistance for College and Higher Education (TEACH) Grant
Used to verify your status as an unaccompanied homeless youth for federal financial aid funding
Used to apply for an unsubsidized direct loan without parent data
