STEM OPT Reporting Form

The Department of Homeland Security (DHS) requires all international students on STEM OPT to report Employment and address changes within 10 days. During the12 month OPT period students are to use the OPT Portal and not this form. However, during a student's OPT STEM EXTENSION period you cannot use the Portal. You are required to report employment information at 6, 12, 18 & 24 months even if the information is the same since the last reporting. Annual self-evaluations from page 5 of the I-983 form are due at 12 and 24 months and you can upload them here. You can also upload a new I-983 if you change employment. Failure to report can cause future immigration issues and/or difficulty with Change of Status to H1B or another status. It is also possible the SEVIS system could automatically terminate your record.

Please use this form to make necessary changes, uploads and provide all your 6-month reports. Once you complete this form, it will automatically be sent to International Student Services, who will report to DHS and your SEVIS Portal electronically.
