    Student Incident Report

    This electronic form may be completed by any member of the university community, alleging a student or student organization has violated student conduct code policies. Reports should be submitted as soon as possible after the event takes place. Upon submission of this report, the director of Student Conduct and Resolution shall conduct an investigation of the allegations. The completed, printed report should be submitted to the Office of Student Conduct and Resolution in Owens Hall 208.
    Release of Student Information This electronic portal is used for students to authorize release of information to other identified individuals regarding a student’s disciplinary file.
    Verification of Community Service Activity This printable form is used for students who are required to complete community service hours as assigned by a student conduct sanction. The completed, printed form, with verification signature at the bottom, should be submitted to the Office of Student Conduct and Resolution in Owens Hall 208, or to the hearing officer the student met with.
    Student Notification Insert This printable document is used regarding the information provided to students with the letter of notification regarding alleged misconduct, conduct code charges and a designated meeting for review.
    Student Conduct Disciplinary Appeal This electronic form is used for students to submit a written appeal after the decision of a hearing officer. The form is submitted to the appropriate decision maker.
