    Once you are admitted to Minnesota State University Moorhead and receive an I-20 for F-1 students or DS2019 for J-1 students, you are entered into an agreement with Minnesota State College and University Systems (MnSCU) and abide by all the statement, policies, regulations and conditions that appears on your I-20 and DS2019 form. Purchasing the MSUM health insurance is mandatory for all F-1 and J-1 students at MSUM. The health insurance cost is stated in item #7 of your I-20 form and stated under item #1 on your DS-2019. This is an annual cost that must be paid during the first few days of your arrival to the U.S. 2016-2017 Annual Health Insurance Cost is $1356.00 August to August Coverage.

    According to U.S. State Department Policy 22CFR514.14 requires that all international visitors on J-1 and J-2 visa carry health insurance provided by the institution.

    Optional Dental Insurance

    International students are able to purchase the optional Dental Insurance plan from United Health Care. The Enrollment date for the Dental plan is before September 20th  and students are responsible to make the payment directly to the Health Insurance Company. MSUM is not responsible for any Dental Insurance benefits or assistance. The total annual cost is $288.24 and must be made directly to the the insurance company. MSUM is not responsible for Dental Insurance. Please see below for helpful links.

    Health Insurance Refund

    All F-1 and J-1 international students and Scholars are required to purchase the mandatory health insurance at Minnesota State University Moorhead. In certain circumstances, F and J students and scholars are eligible to receive a refund. Please see below to see if you qualify:

    • You are transferring to a different U.S. institution
    • You are leaving the U.S. and will not return to MSUM within the next year
    • If you have changed from F or J immigration status you will not be required to purchase student health insurance

    If you believe you qualify for a refund, please submit the Health Insurance Refund Application to International Student Services.
