Accessibility Resources Exam Accommodations

  • I understand that before I take the exam:

    • I must submit the online test form no less than two (2) business days in advance of the day of the scheduled exam/quiz. A business day is defined as Monday through Friday from 8:00 a.m. - 4:30 p.m.
    • All tests must be started early enough that they can be completed by 4:30 p.m.
    • If I am a "no-show" for an exam, the exam will be returned to the instructor and I will need to make arrangements with the instructor to make up the exam. There is no guarantee that the instructor will permit a make-up exam. Accessibility Resources will contact the instructor by email within 24 hours of the scheduled time of the missed exam.
    • I am expected to contact Accessibility Resources as well as my instructor to cancel exams if I am ill, have an emergency, or do not intend to use testing accommodations. This includes withdrawing from a course.
    • Accessibility Resources does not have the authority to reschedule a test date or start time; my instructor must approve that and notify the Accessibility Resources office via email.
    • I need to arrive on time for my exam. I may begin an exam up to 30 minutes prior to the exam’s scheduled start time.
    • If I arrive more than 20 minutes late for the scheduled exam, I will be given the choice of making other arrangements with the instructor or having the number of minutes late deducted from the time allotted for the exam. I am responsible to notify AR of other arrangements made with the instructor.

    I understand the exam rooms at Accessibility Resources have video monitoring equipment. This video surveillance can be used as proof of my actions during testing.

    I understand that while I'm taking the exam:

    • The following items are not allowed in the exam room:
      • Any communication devices, including cell phones and PDAs
      • Notes/books not permitted by the instructor
      • Coats
      • Book bags or purses
      • Hats or ball caps
    • I will not be allowed to use books, notes, or any other form of recorded information unless Accessibility Resources has received special instructions from my instructor indicating that that form of aid is allowable. Attempts to do so without permission will be reported to the instructor.
    • I must abide by the academic policies outlined in the Student Handbook in regard to Academic Honesty in all my dealings with testing accommodation. Any suspected evidence of cheating or use of unauthorized resources will be documented by Accessibility Resources staff and reported to the appropriate faculty member and the Judicial Affairs.
    • A staff member may come into the testing room at any time to perform a random integrity check.
    • I am responsible for monitoring my testing time and completing the exam in the allotted time.
    • I will not be permitted to leave the Accessibility Resources testing area once I have begun my exam.

    If the exam/quiz registration form is submitted less than 2 business days from the time the exam/quiz is to be taken, it will not be accommodated within Accessibility Resources. A business day is defined as Monday thru Friday from 8:00am - 4:30pm.

    If you are unable to complete this form due to your disability, please contact the Director of Accessibility Resources and assistance will be arranged.
