*Seniors in the SLHS program at MSU-Moorhead need to use all three of the attached forms.
*ALL other applicants:* Distribute the first form (rating scale) only to each of your reference writers. Be sure to provide an addressed envelop so that all recommendations are returned to the Graduate Studies Office at MSUM.
