Steps for Completing Internship Agreement Form
*Complete the internship agreement online in the adobe format.
*Print off the form.
*Student and internship supervisor must both sign the agreement form.
*Student should set up an appointment with Deborah Kukowski regarding
the internship. Bring the original signed agreement form to that
appointment to get registered for the internship.
