Safe Environment Program

Each year, everyone in our parish and/or school who works with children, youth or vulnerable adults must complete the following two Safe Environment components that are required by the Diocese of Crookston:

  1. Print out and complete the Acknowledgment and Consent form that applies to you below. A new one of these must be filled out and on file at St. Joseph's Parish each year. Your completed for can be sent to Matthew Devick, safe environment coordinator, at the parish office.

    2. Acknowledgment and Consent Form for Employees
  2. Complete the Safe Environment training. Follow these steps to complete the training:
    1. Step 1: Read through this document: Code of Ethical Standards for Church Leaders (You may either keep this document open in your window's browser or print a copy while you take the test.)
    2. Step 2: Take the test for Component, either online (recommended) or print out a paper version and turn it in to Matthew Devick. The links are found below:
      1. Test for Component - Online test (recommended for faster processing of Safe Environment records)
      2. Paper test for Component (Can be printed out and turned in to Matthew Devick)

Note: In both the online and paper versions of the test, questions 3-10 require, in addition to the correct answer, that you provide the section of the Code of Ethical Standards where the correct answer is found. For example, if you found the correct answer for a question under the section entitled 0.4.1 Human Well-Being, you would type 0.4.1 in the space labeled: Section of the Code____.
