Application forms are available in the Office of International Programs (151 Flora Frick). You may also download the forms here.

To download the form, simply click on the link below. The forms are in Word. You can either type in your information and then print, or print it and then fill in the form. If you type in the information you will need to readjust the spacing as you type. Be sure to mail in the application and 1 recommendation form to International Programs, 151 Flora Frick, Minnesota State University Moorhead, 1104 7th Ave. S., Moorhead, MN 56563.
