Form 5:  Preliminary Graduation Notice


 

  • Students must secure signature on Form 5 from their advisor and the program coordinator to verify that they are eligible to participate in commencement. 
  • In order to participate in May commencement, all requirements for the degree must be completed prior to the end of the summer session. The incomplete degree requirements and completion timeline will be stated on Form 5. Student and advisor will verify these requirements and sign
    Form 5.
  • Students finishing their degree during fall semester will be eligible to participate in the December graduation.
  • Completed Form 5 is sent to the Graduate Studies Office, the Director of Graduate Studies signs the form, and it is recorded and placed in the student's permanent file in the Records Office.
  • Form 5 must be submitted to the Graduate Studies Office six weeks prior to commencement.
