Form 4: Thesis/Project Topic Approval and IRB Approval Verification
- Plan A or Plan B topic is chosen by the student and his/her advisor.
- Student and advisor sign indicating topic approval.
- After proposal is accepted, student, advisor and committee members initial for approval.
- If human subjects are involved in the research, student attaches approval from IRB or DRC (before human subject research begins, approved IRB forms must be on file).
- Completed Form 4 is sent to the Graduate Studies Office, the Director of Graduate Studies signs the form, and it is recorded and placed in the student's permanent file in the Records Office.
