Form 2: Thesis/Project Committee Appointment and Approval
- Curriculum and Instruction graduate students do not submit Graduate Studies Form 2.
- Student and advisor select the committee members.
- All faculty on the committee must have current Graduate Faculty status.
- Printed names and signature of the committee chair and other program committee members are listed.
- The Dean of Graduate Studies may appoint the outside committee member
- Completed Form 2 is sent to the Graduate Studies Office, the Dean of Graduate Studies signs the form, and it is recorded and placed in the student's permanent file in the Records Office.
