Registering for exams to be taken at
Disability Services (DS)
To ensure you will receive the needed
accommodation, submit the on-line test form 7 days in advance of the
scheduled exam/quiz. For example,
for an exam scheduled on a Tuesday, you need to sign up on or before the
previous Tuesday (If your exam is on March 11, you need to sign up on or
before March 4). DS does not have the authority to reschedule a test date
or start time, your instructor must approve that.
- I understand that I must
arrive on time for my exam. I understand that if I arrive more than 20
minutes late for a scheduled exam, Disability Services will not administer
the exam and the exam will be returned to the instructor. I will go to the
classroom to take the exam or make other arrangements with my instructor.
If I begin my test late (up to 20 minutes), the number of minutes will be
deducted from the time allotted for the exam.
- I understand that I must abide
by the academic policies outlined in the Student Handbook in regard to
Academic Honesty in all my dealings with testing accommodations.
- I understand that all tests
must be started early enough so they can be completed by 4:30 p.m.
The following procedural guidelines
apply to tests administered at Disability Services:
- You must arrive on time for
your exam. (For example, if the
class exam is scheduled for 9:00 AM, you must be ready to start the exam at
DS at 9:00 AM). You may begin an exam up to 30 minutes prior to the exam’s
scheduled start time.
- You are responsible for
monitoring your testing time and completing your exam in the allotted time.
- You are expected to contact
DS to cancel exams if you do not intend to use testing accommodations for a
particular exam. If you are a “no
show” for an exam, the exam will be returned to the instructor and you will
need to make arrangements with the instructor to make up the exam.
- You are not permitted to leave
the DS testing area once you have begun your exam (exception is bathroom
break).
- You will not be allowed to
take notes during the exam.
- You will not be allowed to use
books, notes, or any other form of recorded information during the exam
unless the proctor has received special instructions from your instructor
indicating that some form of notes are allowable. Attempts to do so will
be reported to your instructor.
- You must provide all requested
information when completing the on-line registration form. Failure to
complete the form accurately and completely will jeopardize receipt of the
accommodation.
Students authorized for exam
accommodations have three options:
- Take exam with the class.
- Take exams with appropriate
accommodations arranged by the instructor.
- Take exams at DS.
Policies for exams at DS
Faculty instructions for the exam will be reviewed with you before the exam begins.
You will be held responsible for following these instructions at all
times. The following items are not allowed in the exam room:
- Notes/books not permitted by
professor/instructor
- Any communication devices:
including cell phones and PDA’s (which must be turned off)
- Coats
- Book Bags
- Purses
- Hats/Ball caps
You are not
permitted to choose your exam room. DS cannot guarantee a specific test
environment.
You are
responsible for your personal exam materials. If you forget personal exam
materials (calculator) and you leave to retrieve them, when you return you
will only be given the remainder of your allotted time.
If you are
unclear about the exam instructions or conditions, stop the exam and seek
assistance from a DS staff member. The DS staff member will attempt to
contact your instructor or give you a form to complete explaining the problem
or question which will then be returned with the exam.
Policies Regarding Academic
Misconduct
Any
student observed utilizing any unauthorized resource during an exam will be
reported to the Student Conduct Committee through the Student Support
Services Office and their instructor.
Any unauthorized
notes and any scrap paper used during the exam will be copied and returned
with the exam to the instructor.
A staff member
may come into the testing room at any time to perform a random integrity check.
Any suspected
evidence of cheating will be documented by the exam scheduling coordinator
or DS staff and reported to the appropriate faculty member and
the Student Conduct Committee through the Student
Support Services Office.
Pop Quizzes
The
instructor must call DS in the event of a pop quiz as soon as it is planned.
The instructor should deliver the quiz immediately to DS for administration
and enclose instructions for administering the quiz (e.g., time allotment
and authorized materials).
Lateness, Illness, No Show, Cancellation, and Rescheduling Policies
Lateness and
Illness:
You are expected
to be at DS at the time designated and approved by your instructor.
If you arrive
late for your exam, you must take the remaining time or reschedule your exam
with your instructor and then notify DS of those changes.
There is no
guarantee that the instructor will permit a make up exam.
If you are
unable to take an exam due to illness or emergency, contact your
instructor immediately.
You are
responsible for coordinating the makeup of any missed exam or quiz with your
instructor.
No Show:
If you fail to
show up for a scheduled exam, DS will contact you by email within 24 hours.
Your instructor will also receive an email.
Cancelling an
Exam:
If for any
reason you have decided not to take your exam at DS after you filled out the
online exam request form, you are responsible for notifying DS so that we
can have the space for other students. This includes withdrawing from a
course.
Requesting Readers and/or Scribes
for Exams
Readers:
- Readers can be asked to repeat
information, so do not hesitate to ask.
- Readers will only read what is
on the printed page and cannot be asked to interpret, define, explain
or reword questions.
- Readers need feedback from you
to be effective. Let your reader know what reading tone, rate, etc. works
best for you.
Scribes:
- Scribes will write down
verbatim what you have dictated. The scribe is not responsible for
organizing or paraphrasing your thoughts into a final draft.
- Scribes are responsible for
general spelling and sentence ending punctuation. You are responsible for
directing the scribe for any spelling of specific, class-related terminology
or punctuation within sentences.
- At any time, you will have the
opportunity to review what the scribe has written either by reading or
having it read to you.
- If there are
corrections, you will direct the scribe to make them.

If the exam/quiz registration form is submitted less than 2 business days from the time the exam/quiz is to be taken, it will not be accommodated within Disability Services.
A business day is defined as Monday thru Friday from 8:00am - 4:30pm.
