  • Register for an Exam to be Taken at the Disability Resource Center (DRC)

    I understand that before I take the exam:

    • I must submit the on-line test form no less than 2 business days in advance of the day of the scheduled exam/quiz. A business day is defined as Monday thru Friday from 8:00 am - 4:30 pm.
    • All tests must be started early enough that they can be completed by 4:30 pm.
    • If I am a "no-show" for an exam, the exam will be returned to the instructor and I will need to make arrangements with the instructor to make up the exam. There is no guarantee that the instructor will permit a make-up exam. DS will contact the instructor by email within 24 hours of the scheduled time of the missed exam.
    • I am expected to contact DS as well as my instructor to cancel exams if I am ill, have an emergency, or do not intend to use testing accommodations. This includes withdrawing from a course.
    • DS does not have the authority to reschedule a test date or start time; my instructor must approve that and notify the DS office via email.
    • I need to arrive on time for my exam. I may begin an exam up to 30 minutes prior to the exam’s scheduled start time.
    • If I arrive more than 20 minutes late for the scheduled exam, I will be given the choice of making other arrangements with the instructor or having the number of minutes late deducted from the time allotted for the exam. I am responsible to notify DS of other arrangements made with the instructor. 

    I understand the exam rooms at the Disability Resource Center have video monitoring equipment. This video surveillance can be used as proof of my actions during testing.


    I understand that while I'm taking the exam:

    • The following items are not allowed in the exam room:
      • Any communication devices, including cell phones and PDAs
      • Notes/books not permitted by the instructor
      • Coats
      • Book bags or purses
      • Hats or ball caps
       
    • I will not be allowed to use books, notes, or any other form of recorded information unless DS has received special instructions from my instructor indicating that that form of aid is allowable. Attempts to do so without permission will be reported to the instructor.
    • I must abide by the academic policies outlined in the Student Handbook in regard to Academic Honesty in all my dealings with testing accommodation. Any suspected evidence of cheating or use of unauthorized resources will be documented by DS staff and reported to the appropriate faculty member and the Judicial Affairs.
    • A staff member may come into the testing room at any time to perform a random integrity check.
    • I am responsible for monitoring my testing time and completing the exam in the allotted time.
    • I will not be permitted to leave the DS testing area once I have begun my exam.

    If the exam/quiz registration form is submitted less than 2 business days from the time the exam/quiz is to be taken, it will not be accommodated within the Disability Resource Center. A business day is defined as Monday thru Friday from 8:00am - 4:30pm.
