Reporting Instructions
Please follow these instructions
carefully to ensure that you file the correct information with the Student
Learning Outcomes Assessment Committee. If you have questions, please read
the committee’s
policies or contact Dr. Jean
Sando at
sandoje@mnstate.edu.
Accredited Programs
If your program is accredited by an external
agency or organization, file the Accredited Programs Cover Sheet by October
31, 2007 (see instructions below).
When saving the form,
replace “program” in file name with appropriate
program name (abbreviated).
Non-Accredited Programs

Do you wish to make changes to your student
learning outcomes or assessment plan? If so, file an Assessment Plan Cover
Sheet and Assessment Planning Form for each outcome you are revising.
Assessment plans may be changed at any time (see filing instructions below).
(You can review your assessment plan by clicking the
Plans and Reports link
on the assessment website. You do not have to file Assessment Planning Forms
for outcomes that will be assessed in the same way as in previous years.)

program name (abbreviated) and add number of student learning outcome
addressed by the form.

All non-accredited programs will
file an Assessment Report Cover Sheet and an Assessment Reporting Form for
each outcome on which you are reporting by October 31, 2007 (see
instructions below).

All Programs
If you wish to apply for an Instructional
Improvement Grant to enhance student learning, file the Instructional
Improvement Grant Application by January 31, 2008.

Filing Instructions

Print all required forms.

Obtain signatures on all
required cover sheets of department chair/program coordinator and dean.
Include records of department discussion and approval of the assessment
documents.

Send paper copies of all
forms to Jean Sando, OW 206A.

Email electronic copies of
all forms (without signatures) to assess@mnstate.edu

Deadlines: October
31, 2007 for assessment reports and January 31, 2008 for Instructional
Improvement Grants.
Note: A “program” is a
curriculum that leads to a baccalaureate, master’s, specialist, or doctoral
degree. Minors and certificates are not required to file assessment plans
and reports. Apply for an
Instructional Improvement Grant -- Deadline is January 31,
2008. |
