Instructions
- The APAC Signature Sheet/Checklist must accompany all APAC proposals along with the appropriate form (please see list below). Signatures must be obtained from the department chairperson/director, Dean, and the appropriate curriculum committee.
- The Department Chairperson or department representative(s) should attend the APAC meeting their item is scheduled to be discussed.
There are two ways to submit a proposal for placement on an APAC agenda:
Electronically: E-mail all documents to apac@mnstate.edu . Send the signed original proposal to Academic Affairs. (submission deadlines are indicated above).
Hardcopy: Submit the signed original and 27 copies of the proposal to Academic Affairs (submission deadlines are indicated above).
