Academic Planning and Reporting Instructions and Forms
1. All departments/programs should download the following document:
2. All departments/programs must file a Student Learning Outcomes Assessment Report. Forms are available from the Student Learning Outcomes Assessment Committee site. These reports are due October 31, 2008.
Accredited programs must file the Accredited Programs Cover Sheet and any assessment report due their accrediting agency.
Non-accredited programs must file a full MSUM assessment report.
3. Departments/programs that are not writing a six-year self-study should download the Department Work Plan guidelines (and necessary forms) below. Deadline for the Department Work Plan is May 1, 2009.

Digital Measures - Digital Measures will be replacing the Delaware Study Summary Form. Information will be forthcoming during Fall semester 2008
